Important End of Year Reminder from the Health Office

Any medication kept in the health room (prescription and/or non-prescription) needs to be picked up by the last day of school (Thursday, June 13th). Medications will not be handed out to students to bring home. Per District policy, all medications must be picked up by a parent or another responsible adult. After June 20, any medications left at school will be destroyed.

A new Parent/Physician Medication Form is required at the start of a new school year. This form is available on the district home page under “Health Services” in the Parents tab.

This consent form can be completed at summer doctor appointments (a doctor’s signature is required for all prescription medications including inhalers, epipens, etc). Medications for the new school year can be brought to our Back to School event in August. Please plan ahead to have a hassle free start to the new school year!

Questions? Call the DPMS Health Secretary at 983-9174 x2016.

HAVE A SAFE AND HAPPY SUMMER! SEE YOU NEXT YEAR!!
